
SOP 8: Employee Wellness Program – Annual Physical Initiative
1. Purpose
The purpose of this Standard Operating Procedure (SOP) is to promote preventive health and overall well-being among employees at E.C. Fennell, Inc. (“ECF”) through the implementation of a structured Annual Physical Wellness Initiative. This program encourages employees to complete an annual physical examination with their healthcare provider and receive a small but meaningful incentive for participating.
This initiative aligns with ECF’s commitment to fostering a healthy workforce, managing long-term healthcare costs, and supporting employees’ personal health goals.
2. Scope
This policy applies to all full-time employees who have successfully completed 90 days of continuous employment with ECF.
3. Policy Statement
ECF recognizes the value of preventive healthcare and believes that regular physical examinations are key to early detection of potential health risks and long-term employee well-being.
Eligible employees who provide documentation verifying completion of an annual physical exam within the current calendar year will receive a Wellness Incentive as outlined below.

Annual Physical Wellness Initiative
4.2. Eligibility Criteria:

Full-time employees only.

Must have completed at least 90 days of employment.

Physical must be completed within the same calendar year of the incentive request.
4.3. Incentive Options:
Upon submission and verification of documentation, eligible employees will receive the following incentive:

One $25 gift card.
4.4. Incentive Rules:

Only one incentive may be earned per calendar year.
5. Documentation Requirements
Employees must submit one of the following forms of verification to HR:

A signed Annual Physical Verification Form Important:
Medical information or diagnostic results must not be submitted to ECF.
