I, the undersigned, acknowledge and confirm that the requested rental vehicle will be used exclusively for authorized E.C. Fennell, Inc. business purposes only. I agree to operate the vehicle in accordance with company policy and applicable motor vehicle laws and regulations.

I understand that personal use of the vehicle is strictly prohibited, and that failure to comply may result in disciplinary action and/or termination of vehicle privileges.

Additionally, I confirm that a copy of the finalized rental agreement will be submitted to E.C. Fennell Human Resources via email at ECFHR@ecfconsultants.com upon receipt as part of the compliance process.
